Your policy name matters

Network dental, network vision and supplemental dental are different products. Start with the policy or member identification card. The correct insurer, administrator and network name help you avoid using the wrong directory.

Verify before an appointment

• Search using the network shown on your own policy.
• Confirm the provider’s current participation with the plan.
• Ask about eligible services, pre-treatment estimates and personal costs.
• Check deductibles, annual maximums, allowances and frequency limits.
